Stony Brook University Hospital offers both paid and unpaid employee leaves of absence for medical conditions related to the employee or family member and for maternity/paternity leave.

FLMA Forms:  

NYS Paid Family Leave (PFL) Forms for MC and UUP employees ONLY:

Paid Parental Leave (PPL):

Paid Parental Leave will be granted to qualified employees for 12 weeks with no charge to accruals once every 12 months.  PPL is available for the birth of a child, adoption, and for foster care placement.  PPL may begin on the child/children's date of birth, date of adoption, date of foster care placement, or anytime thereafter within seven months of the qualifying event.  PPL eligibility ends seven months from the date of the qualifying event and must be used consecutively (PPL cannot be used intermittently). Employees are not required to utilize the full 12 weeks; however, once an employee returns to work, the PPL concludes.

Parental Leave Information

Leave Donation Program:

Employees in good standing who are on an approved medical leave of absence for their own illness, who have exhausted all of their accruals and expect to be on leave for at least two pay periods (one pay period for UUP members), may apply for leave donations.  If you wish to request donations while you are on leave or if you wish to donate some of your annual accruals to help a coworker, please visit: Human Resources Forms | ThePulse (stonybrookmedicine.edu) or you can email Leaves@stonybrookmedicine.edu for more information.
